A raft of extra security measures are being introduced at a hospital where a mental health patient fell to his death seven months ago.

But the mental health trust responsible for the care of Adam Withers, 20, who plunged from a tall chimney at Epsom Hospital, has still not released the findings from an investigation into how he died.

Following his death on May 9, his grieving friends called for urgent action to prevent a repeat of the tragedy.

At the time Surrey and Borders Partnership Trust (SABP), which runs mental health wards at the hospital, said a report from its investigation would be completed within 60 working days.

Last week a spokeswoman at the trust said: "We have finalised our investigation but it does need to go to the scrutiny committee.

"The scrutiny process is two step – our own internal and then an external panel made up of our commissioners. This will happen over the next two months.

"We would discuss with the family before releasing information to the press."

Asked what extra measures had been introduced to try and stop any further tragedies at the site a spokesman said: "These include the protection of drainpipes and ledges to remove the ability for someone to lever themselves out of the internal courtyard.

"A roll bar is also being installed to ensure there is no sufficient grip to enable someone to climb or pull themselves up." The trust said work is also being carried out on "external gardens" which is due to be completed in January.

A spokeswoman at Epsom and St Helier Hospitals Trust spokeswoman said they have also made changes to make sure other patients cannot reach the chimney.

She said: "On the main entrance side, we have increased the height of the gates and installed an access control system, so only authorised people can access via the normal route.

"In regards to the other side, we put up a temporary wall which was much higher than the previous one.

"We are now in the process of installing a new fence and gate in its place, which again will prevent anyone access through a normal route.

"The later has not yet been completed, as the area has become a point of remembrance and we would want the family's approval before moving anything."

Health watchdog for Surrey said the mental health trust has not given them any information on what action has been taken in the wake of Adam’s death.

Its engagement and communications manager, Jane Shipp, said: "Healthwatch Surrey has not received any information or report on the investigation of Adam’s fall.

"We will be asking SABP for an update imminently."
